How to Become a Ghoul in Fallout 76 and What It Changes
Requirements, benefits, drawbacks and important warnings to understand before transforming your character.
How to Become a Ghoul
To take the plunge and transform your character, you'll first need to reach level 50. Once you do, the Leap of Faith quest will unlock in the Whitespring Refuge area, which is what allows you to start the transformation.
Core Changes & Mechanics
Becoming a ghoul removes the hunger and thirst meters entirely and replaces them with two brand-new systems:
1. The Feral Meter (Sanity State)
Replaces hunger and thirst. It represents your internal struggle to keep your sanity and prevent yourself from turning into a feral ghoul.
* How it drains: It takes roughly 2 hours of real-time play to fully empty (about 1% every 2 minutes).
* How to fill it: It is replenished exclusively by consuming chems/drugs (such as Mentats, Psycho, Buffout, or Glowing Blood).
* States and effects:
* High Sanity (76% - 100%): You gain passive buffs (+3 Strength, +3 Endurance, and +30 Max Health).
* Mid/Low Sanity: You progressively lose Endurance, Max Health, and Action Points (AP).
* Fully Feral (0%): You suffer severe penalties (-99 Charisma, -300% VATS/hip-fire accuracy), but gain a massive +150% melee damage. Certain perk cards only trigger in this state.
2. The Glow Meter
Functions as a fuel gauge and a biological energy shield overlaying your health bar.
* How to fill it: Absorbing radiation (rad zones, irradiated food, or dirty water). Instead of harming you, radiation heals your health and fills your Glow meter.
* How it gets used: It acts as a secondary health pool that absorbs incoming damage before it hits your actual health. Additionally, several ghoul perks consume your Glow as ammo/energy to grant extra damage, speed, or stagger immunity.
* How to reduce it manually: Using RadAway or decontamination showers will clear your Glow. Keep in mind that higher radiation resistance on your armor will slow down how fast this meter builds up.
3. Faction Hostility & Disguises
Keep in mind that certain factions will shoot on sight once you're a ghoul. To complete affected quests without turning everyone hostile, you'll need to wear disguises provided by Jaye.
Perk Card Rework
When you transform, you'll lose access to human perks focused on digestion, disease resistance, or radiation mitigation, and gain access to perks centered around mutations, Glow, and managing your feral state.

Is It Worth It?
This path is best suited if you're looking for a fresh gameplay style:
* You want to run dedicated radiation or Glow builds.
* You're looking to break away from the traditional low-health human meta (Unyielding / Bloodied).
* You want to experiment with a new set of perks and don't mind managing constant chem consumption to keep your sanity in check.
If you prefer keeping your current setup, no worries: it isn't required to access any standard Fallout 76 content.
Important Considerations Before Converting
You can switch back to being human at any time via the character menu, but keep a few caveats in mind:
1. The intro quest is a one-time deal. If you revert to human and decide you want to go back to being a ghoul later, you'll have to use the re-transformation feature purchased with Atoms.
2. Ghoul-exclusive perks are disabled whenever you are in human form.
Pro tip: Save a copy of your current human build at a Punch Card Machine before pulling the trigger. That way, you can easily compare setups or swap back without losing your previous loadout.
